Conemaugh Memorial in Johnstown and UPMC Bedford received the top grade of "A" for fall 2017.
Leapfrog gave letter grades to some 2,630 hospitals across 50 states plus D.C. based on a range of patient safety measures. Its hospital safety grades, released every April and October since 2012, are aimed at encouraging patients to pay attention to safety when they choose hospitals. Other states with significant improvement include OR, going from 48th in 2012 to eighth today, Hawaii from 36th to third, Wisconsin from 44th to sixth, and Idaho from 19th to fourth.
Virginia Mason has earned an A grade 12 consecutive times and is one of only 59 organizations in the United States that has achieved straight-A status in the Leapfrog program.
That's down from 25 MI hospitals with an "A" grade in spring 2017.
Memorial Hospital of South Bend, Lakeland Regional Medical Center in St. Joseph, and Parkview Regional Medical Center in Fort Wayne all received A grades. Sixty percent of hospitals in the state got A grades from the national health care nonprofit Leapfrog, which studied data from the American Hospital Association and other sources.

WellStar's Douglas and Paulding hospitals were two of just 14 hospitals in the state to receive an "A" ranking.

Binder said states with consistently good hospital grades show a state-wide, collaborative commitment to patient safety.
An annual grading system says Michigan's hospital care rank is dropping. But the state agency has a full consumer report of all the in hospitals available on its website.
In the category of infections for the fall 2017 report, Beaufort Memorial Hospital scored as well as the best hospitals in the country all five areas surveyed, which was one more category than the spring report.
